Preparing for Your First Appointment
When you go to your first appointment, bring the the below documents to help the intake process run smoothly and ensure you get what you need.
If you don’t have or can’t find all of them, do NOT let that stop you from making an appointment! Let the organization know and they will help you find a solution.
Here’s one resource on how to replace lost, damaged, or stolen personal documents.
Here’s what you should bring:
Proof of Income (one of the following)

Current pay stub

Current tax forms (federal or state income tax returns)

Current Department of Labor letter

Written statement from an employer on letterhead stationary

Copy of unemployment benefits check

Verification of worker’s compensation

Prison release paper (within 30 days of release)
Proof of HIV Status (one of the following)

Copy of reactive HIV test result, from an HIV testing site

Official lab results with the HIV viral load, (with your name clearly indicated)

Written documentation of HIV or AIDS diagnosis, signed and dated by a nurse practitioner or doctor
Copy of any of the following if you have them (insurance is NOT required to access care)

Food Stamps award
-
Medicare/Medicaid card
-
Private insurance card
List of Current Medications
-
Name and dosage of any current medicine (you can bring in the prescription bottle if that’s easiest for you)
Any questions that you have!

What do you want answered by the doctor? The case manager?

Make a list and bring it with you so you don’t forget anything!
